The care of the older patient with Parkinson's disease came into the spotlight with the publication of both National Service Frameworks for Long-term Conditions and Older People. This was a major step forward in ensuring all patients had access to high quality, integrated care, tailored to their needs. The onus is now on local health and social care organisations to put this at the forefront of their local plans. In September 2003, the Primary Care Task Force of the Parkinson's Disease Society produced the third edition of `Parkinson's Aware in Primary Care'1, This evidence-based guideline for GPs recommends early specialist referral and the Parkinson's disease nurse specialist as a key worker. Avion Douglas and Anser Qureshi undertook an audit looking at care of patients with Parkinson's disease in Chelmsford, to determine compliance with this best practice guideline. Patients with suspected or confirmed idiopathic Parkinson's disease were identified from random GP registers within Chelmsford Primary Care Trust. Data was collected on a standard form, by retrospective review of GP and hospital records. The referral rate to local Parkinson's services in Chelmsford is 77%, which is above the UK average of 50%. More importantly the results demonstrated a lottery of care in referrals for specialist care. The very elderly, disabled, cognitively impaired, institutionalised or those requiring social care were the most vulnerable. They are less likely to be referred to specialist services, have the worst outcomes, the highest economic burden, yet would benefit the most from comprehensive Parkinson's disease specialist services supported by a specialist nurse and the multidisciplinary team. However, also the low treatment group, with doses comparable to what infants get after cataract surgery, had significantly impaired growth and bone parameters, why we think that care should be taken when treating infants intensively with glucocorticoid eye drops. In another study looking at growth impairment of glucocorticoids in systemically treated rabbits, when treatment was finished, catch-up growth was observed Gafni et al. 2001 ; . Further studies are needed to investigate catch-up growth in the current animal model and thorazine.

Ms Smith said that she and Kenneth both learned a lot from these classes and that they had planned to incorporate many of the things they learned into their own dealings with the children. All this to suggest that this incident and this conviction are not evidence of any backsliding in Kenneth's dedication to change his life and be a responsible parent and provider.
