Non-clinical pharmacology and toxicology Preclinical data revealed no special hazard for humans based on conventional studies of safety pharmacology, repeated dose toxicity and genotoxicity. There are no studies on the carcinogenic potential of natalizumab. However, as regards reproductive potential adverse effects, pre-clinical reproductive studies of natalizumab suggest the possibility of an increase in abortion rate. The effect is consistent with the known role of 4-integrins in fertilization and implantation. Natalizumab is immunogenic in almost all animals within 2-4 weeks depending on the administered dose. Efficacy The CHMP considered clinical efficacy not sufficiently proven and the efficacy database not sufficient to unequivocally demonstrate efficacy of natalizumab in Crohn's Disease CD ; . The deficiencies pertained to the fact that the primary analysis of the induction study CD301 did not reveal a statistically significant nor clinically relevant effect of natalizumab and that the re-defined efficacy population for the subsequent maintenance study CD303 was not justified since no significant effect of natalizumab was demonstrated in CD301. Subgroup analysis of study CD301, based on a post-hoc calculation of efficacy in a patient population with elevated C-reactive protein CRP, a marker for ongoing inflammation ; was not considered acceptable as proof of efficacy. Using the outcomes from study CD301 and exploratory analyses, the applicant has meanwhile performed a completely new study for induction therapy CD307 ; . This study confirms the findings of the post-hoc analysis of study CD301, however, the effect size indicates clinically relevant although not outstanding efficacy for the induction of remission. Lack of prospective data in maintenance treatment in the intended indication The population studied in CD303, the maintenance study that had already been part of the initial application, is not the population that will be eligible for treatment. The applicant did not perform a maintenance study subsequently to the newly performed induction therapy study CD307. Therefore, there are only results from one single pivotal trial for maintenance therapy of CD, which have been analysed according to the new indication claimed post-hoc. Due to the fact that CD301 which enrolled patients to be later eligible for enrolment in study CD303 ; did not enrol patients based on the intended indication claim, the subsequent patient population in the only pivotal trial for maintenance treatment is not representative. This is not considered sufficient as proof of efficacy, as results are not outstandingly efficacious. Applicability of the results to the intended population has to be demonstrated prospectively. The fact remains that data on the true population for which treatment is envisaged are sparse, and a confirmatory trial is considered necessary Safety Reduced lymphocyte surveillance as induced by 4-integrin antagonism by natalizumab might have been causative to the occurrence of 2 cases of progressive multifocal leukencephalopathy in patients with MS, and a further case in a patient with Crohn's disease. Two of the cases one in the MS trials and the CD patient ; were fatal. The current safety database does not yet allow for a clear estimation of the risk of serious and or fatal adverse events, like PML or other serious infections. Based on these considerations and a considerable efficacy in Relapsing Remitting Multiple Sclerosis RRMS ; , the product Tysabri, containing natalizumab, was given a marketing authorisation, but for a highly restricted patient population. However, unlike patients with RRMS, patients with CD are commonly immunosuppressed, also often in case of formal "wash-out" of previous treatments. This makes treatment with natalizumab highly problematic as regards the possible occurrence of severe opportunistic infections including PML. Regarding opportunistic infections other than PML, there is an elevated risk, for instance pulmonary infections were more common in the CD patients treated with natalizumab, and organisms were isolated that are predominantly seen in immunosuppressed individuals.

All patients studied had abnormal examinations. The abnormalities were most severe at the bifurcation of the CHD, and all patients had segmental involvement at this site. While the disease process was identified at branch points within the biliary tree, it was not confined to them. No patients had identifiable abnormalities of the distal common bile duct. We have classified the distribution of lesions into four groups. Group 1 patients had disease limited to the bifurcation of the CHD 6 of 1 fig. 1 ; . We divided the radiographic findings into four grades of severity according to the classification of Chen et al. [5]. Grade 1 lesions appeared as mild serrations or beading of the luminal contour 6 of 23 ; fig. 4 ; . Grade 2 lesions appeared as mild, smoothly tapered stenosis of the luminal caliber without proximal duct dilatation 3 of 23 ; fig. 5 ; . Grade 3 lesions appeared as moderate 50%-80% ; stenosis with mild dilation proximally 7 of 23 ; figs. 3A and 3B ; . Grade 4 lesions had severe to subtotal 80% 99% ; irregular segmental stenoses with definite dilatation of proximal ducts 7 of 23 ; fig. 5 ; . Several patients had more than one pattern of involvement at different areas. Because of persistently elevated liver-function tests and increasing right-upper-quadrant pain, one patient had ERCP at the end of two and six cycles of IA-FUDR therapy. The examinations revealed progressive sclerosis Grades 1-3 ; limited to the CHD bifurcation Group 3 ; fig. 6 ; and the cystic duct. Cholecystectomy performed 1 week after the second ERCP revealed a densely fibrotic cystic duct and gallbladder, acalculous cholecystitis, and no evidence of tumor within the porta hepatis. Three other patients studied earlier developed symptoms of the right upper quadrant and liver-function tests suggestive of chemical cholecystitis and were shown operatively to have.
There are significant though modest relationships of MRI lesion load and activity in CIS and relapsing MS with relapses and disability16-18. Localized disease which extended to the pleural surface. The diagnoses obtained as a result of needle biopsy were in most cases confirmed by postmortem examination or the subsequent course of events, although in some cases the actual diagnosis is still somewhat speculative. In two cases, no lung tissue was obtained. On one occasion, opening the needle chamber resulted in aspiration of a large amount of blood. Since the patient was suspected of having a nonresolving pneumonia, it was felt that the needle had entered some very vascular pleural or pulmonary tissue. Therefore, without taking a biopsy, the needle was withdrawn. Neither hemoptysis nor hemothorax followed. The patient's subsequent benign course would tend to confirm the suspected diagnosis. The second case was one in which it was suspected that the visceral pleura was so thick that it completely occluded the biopsy chamber, so that a lung specimen was not obtained despite repeated attempts. This was confirmed at subsequent open lung biopsy, when the pleura was noted to be approximately 1 cm thick, overlying lung which showed evidence of severe fibrosing alveolitis. There was no evidence of asbestos exposure in this case. In two other patients, the lung biopsy was misleading. Both showed histologic evidence of nonspecific inflammatory reaction. However, one later turned out to have active pulmonary tuberculosis, with a positive acid-fast smear of the sputum, while the other patient had Blartomyces dermatitidis cultured from skin lesions, although no evidence of fungal infection could be obtained from lung biopsy material. Table 1 summarizes clinical and histologic information, as well as the incidence of pneumothorax. Of the 44 biopsies performed in 42 subjects, 42, or 96 percent, were successful. There was a 28 percent incidence of pneumothorax, although only six, or 14 percent, of patients required a chest tube and in only one of these was it necessary to leave it in place for over 48 hours. No other complications of any kind were noted and natrecor.

On 23November, FDA licensed a new biologic approach for patients with relapsing forms of multiple sclerosis MS ; to reduce the frequency of symptom flare-ups.
