Mycobacerium avium complex The most common non-MTB organisms responsible for pulmonary and disseminated disease are part of MAC, which includes both M. avium and M. intracellulare. More than 90% of MAC isolated from AIDS patients is M. avium. It occurs mainly in PLHA with CD4 100.205 MAC has been isolated from a variety of sources around the world, including soil, natural water, municipal water system, food, house dust, wild & domestic animal. Disseminated infection results from recent infection ingestion or inhalation ; rather than reactivation of a previous infection. MAC infection has not clearly emerged as a problem in the developing world where Mycobacterium tuberculosis appears to be the predominant pathogen.201 It is speculated that acquired immunity against mycobacteria through previous infection with tuberculosis or through vaccination with BCG provides protection against MAC. Another possible explanation is the difficulty to diagnose MAC in resource-poor settings, and the short life expectancy of patients with HIV infection. In the MSF home care project in Bangkok, out of 601 patients between 1995 and 2000, 3.3% had MAC confirmed by blood culture. In several recent studies from Africa and Thailand disseminated MAC was diagnosed in 5-10% of febrile hospitalised HIV-patients with low CD4 counts, indicating that under-diagnosis plays a factor in the low incidence of MAC in an African setting.42, 206, 207 One study from Spain showed MAC is the third aetiology of splenic abscesses 15% ; after tuberculosis 43% ; and visceral leishmaniasis 21% ; .197 Symptoms and signs Typically MAC presents with prolonged fever, gradual wasting, severe anaemia and neutropenia. The CD4 count is usually less than 30. MAC infection frequently involves gastrointestinal tract, liver, abdominal lymph nodes and spleen. More than 70% of patients have gastrointestinal or hepatobiliary symptoms: diarrhoea, abdominal pain, hepatomegaly, and raised alkaline phosphatase more than 2 x ULN ; and gamma GT levels more than 3 x ULN ; . When compared with disseminated TB, MAC patients have absence of peripheral lymphadenopathy, and more severe anaemia and neutropenia. Fulvin strongly suppressed the steady-state dynamic instability of MAP-free bovine-brain MTs in vitro at concentrations well below those required to reduce the polymer mass. Significant suppression of the shortening rate and the catastrophe frequency at the plus ends occurred at griseofulvin concentrations as low as 0.5 M, when, based on the affinity of griseofulvin for tubulin, very little soluble tubulin could have been bound to the drug 1 and mefloquine. Pregnancy: Pregnancy Category C. Reproduction studies with dronabinol have been performed in mice at 15 to 450 mg m2, equivalent to 0.2 to 5 times maximum recommended human dose MRHD ; of 90 mg m2 day in cancer patients or 1 to times MRHD of 15 mg m2 day in AIDS patients, and in rats at 74 to 295 mg m2 equivalent to 0.8 to 3 times MRHD of 90 mg m2 in cancer patients or 5 to times MRHD of 15 mg m2 day in AIDS patients ; . These studies have revealed no evidence of teratogenicity due to dronabinol. At these dosages in mice and rats, dronabinol decreased maternal weight gain and number of viable pups and increased fetal mortality and early resorptions. Such effects were dose dependent and less apparent at lower doses which produced less maternal toxicity. There are no adequate and well-controlled studies in pregnant women. Dronabinol should be used only if the potential benefit justifies the potential risk to the fetus. Nursing Mothers: Use of MARINOL Capsules is not recommended in nursing mothers since, in addition to the secretion of HIV virus in breast milk, dronabinol is concentrated in and secreted in human breast milk and is absorbed by the nursing baby. Geriatric Use: Clinical studies of MARINOL Capsules in AIDS and cancer patients did not include the sufficient numbers of subjects aged 65 and over to determine whether they respond differently from younger subjects. Other reported clinical experience has not identified differences in responses between the elderly and younger patients. In general, dose selection for an elderly patient should be cautious usually starting at the low end of the dosing range, reflecting the greater frequency of falls, decreased hepatic, renal, or cardiac function, increased sensitivity to psychoactive effects and of concomitant disease or other drug therapy. ADVERSE REACTIONS Adverse experiences information summarized in the tables below was derived from well-controlled clinical trials conducted in the US and US territories involving 474 patients exposed to MARINOL Capsules. Studies of AIDS-related weight loss included 157 patients receiving dronabinol at a dose of 2.5 mg twice daily and 67 receiving placebo. Studies of different durations were combined by considering the first occurrence of events during the first 28 days. Studies of nausea and vomiting related to cancer chemotherapy included 317 patients receiving dronabinol and 68 receiving placebo. A cannabinoid dose-related "high" easy laughing, elation and heightened awareness ; has been reported by patients receiving MARINOL Capsules in both the antiemetic 24% ; and the lower dose appetite stimulant clinical trials 8% ; . See Clinical Trials. ; The most frequently reported adverse experiences in patients with AIDS during placebo-controlled clinical trials involved the CNS and were reported by 33% of patients receiving MARINOL Capsules. About 25% of patients reported a minor CNS adverse event during the first 2 weeks and about 4% reported such an event each week for the next 6 weeks thereafter. DRUG ABUSE AND DEPENDENCE MARINOL Capsules is one of the psychoactive compounds present in cannabis, and is abusable and controlled [Schedule III CIII ; ] under the Controlled Substances Act. Both psychological and physiological dependence have been noted in healthy individuals receiving dronabinol, but addiction is uncommon and has only been seen after prolonged high dose administration and marinol.
